Responsibilities:
- Conduct assessments and develop individualized service plans.
- Provide ongoing case management, advocacy, and linkage to community resources.
- Coordinate care with providers, agencies, and natural supports.
- Monitor client progress and adjust service plans as needed.
- Maintain accurate documentation in compliance with state and agency standards.
Qualifications:
- Bachelor’s degree in human services (social work, psychology, counseling, or related field).
- A year of work experience after receiving college degree.
